These are the details for the person who we will care for when they pass away.

Please provide the basic information requested below to enable a prepaid funeral to be established.  Next of kin is the person who we will be talking to when a death occurs. For details of monthly instalment payments and any deposit required we will call you once the form is submitted.

Once submitted paperwork will be prepared within 24 hours and sent to the nominated address provided above.

* This is applicable to all prepaid funerals paid by 36 monthly instalments. 

How we use your information

Once you have applied for a funeral plan, we will process your personal information for the purpose of administering the funeral plan, which may involve contacting you with relevant information.

How we share your information

We will never sell or share your information with any third parties for marketing purposes. Your information will only be shared with Australian Unity FPM for the purposes or arranging a prepaid funerals and carrying out a funeral service
